Care Management
Families should be able to focus on their child’s well-being rather than spending their time trying to navigate multiple systems on their own. Care Management helps children and families coordinate healthcare, behavioral health, educational, and community-based services so the right supports can work together around the family.
Care managers partner with families to understand their goals, develop individualized plans, connect them to services and resources, advocate when barriers arise, and coordinate among providers. The goal is to create more consistent support, reduce gaps in care, and help families feel more confident navigating the systems that affect their child’s health and stability.
Care Management is provided as part of New York State's Children's Health Home program.
